What form should a government use to fulfill its annual financial reporting requirements?
There are seven formats available for a unit of government to fulfill their annual financial reporting requirements. Please carefully read each description to ensure that you are using the correct form.
- Comptroller Connect Internet Filing - The easiest and most efficient way to file an Annual Financial Report [AFR]. A customized form is created for each unit of government based on the completion of a series of administrative questions.
- Multi-Purpose Long Form - Municipalities, counties, and townships that utilize funds in addition to general or special revenue funds or that have assets should use this form.
- Multi-Purpose Short Form - Municipalities, counties, and townships that only have general and special revenue funds, that operate on a cash basis, and that record no assets should use this form.
- Special Purpose Long Form - Single/special purpose governments that utilize funds in addition to general or special revenue funds or that have assets should use this form.
- Special Purpose Short Form - Single/special purpose governments that only have general and special revenue funds, that operate on a cash basis, and that record no assets should use this form.
- Special Purpose Abbreviated Form - Single/special purpose governments that collect less than $100,000 in revenue, that operate on a cash basis, and that do not record debt should use this form.
- Verification of Appropriation Form - Single/special purpose governments that appropriate less than $6150 for FY 2007, $6005 for FY 2006, $5800 for FY 2005, $5625 for FY 2004, $5520 for FY 2003, $5475 for FY 2002, $5400 for FY 2001, and $5217 for FY 2000 should use this form.
